Abstract
Background: Chylothoraxisanuncommon(3–8%risk)butpotentiallyfatalcomplicationofesophagectomywithpoorlyunderstoodriskfactors.Ithasahighmorbidityduetolossoffluids,electrolytes,andothernutrients,lossoflymphocytesandimmunedysfunction.Methods:Retrospectivechartreviewofadultpatientswhounderwentesophagectomybetween2009and2016wasperformed.Caseswithchyleleakwereidentifiedaccordingtoasetcriteria.Clinicalfeatures,operativefindingsandpostoperativevariableswererecordedandpredictorsofchyleleakwereanalyzed.Results:Duringthestudyperiod,atotalof193adultpatientsunderwentesophagectomyofwhich186receivedneoadjuvantchemotherapy.Themeanagewas53yearswith118malesand74females.Typeofprocedureperformedwas3-stageesophagectomyin98,Transhiatalesophagectomyin79andIvor-Lewisesophagectomyin15patients.Chyleleakwasidentifiedin9(4.6%)patients.Therewasnosignificantassociationofchyleleakwithage,gender,co-morbid,leveloftumor,Neo-adjuvanttherapyandTypeofesophagectomy.Chestdrainoutputonpostoperativeday5wassignificantlypredictiveofchyleleak(p-value<0.05).Drainoutputmorethan1000onday4washighlysuggestiveofchyleleak(p-value<0.05).Dayonwhichchestdrainwasremovedwasalsofoundtobesignificantlyrelatedtochyleleak(p- lue<0.05).Conclusion:Nosignificantpreoperativeriskfactorswereidentifiedforchyleleak.Highchestdrainoutputonpostoperativeday5anddrainoutputmorethan1000onday4aresignificantpredictorsofchyleleak.
